About the fee, plainly

There is one. The number is not published, and that is deliberate.

It varies with the event, the audience, the travel and the preparation involved, so it gets quoted rather than posted. What is worth saying openly is that a fee exists, because otherwise every organization assumes the answer is free.

A significant amount of this work is done at a reduced rate or at no charge, particularly for schools and non-profits. If you are one of those, ask anyway. Where the time is donated, the value is recorded as a donation in kind.

The honest version

If you are a school, ask. It is very often free.

If you are charging admission to hear the talk, the fee applies.

Interviews, podcasts, documentary and expert comment

Available for science journalism, hyperbaric and diving-safety comment, and brain injury coverage, with the standing caveat that Joe is a biomedical engineer, not a physician, and will not be quoted as one.

All requests route through Joe’s team rather than to Joe directly. It is faster that way, and it is how he wants it.
